The following files are printable and (in some cases) modifiable versions of forms in the book.  You may copy and modify them for your own use. You may not sell them or distribute them to other people without our permission. We appreciate your adherence to these copyright constraints.

Some forms are given in several formats. The .pdf format is provided because it ensures the most reliable reproduction of the form and its format. The .doc (Microsoft Word) and .xls (Microsoft Excel) formats are provided because they are easily modifiable if you own those applications.

If simply clicking on a link does not download it, try either clicking with the right mouse button (on a Windows computer) or Control clicking (on a Macintosh computer) and then choosing the appropriate option (which, depending on the browser you are using, may appear variously as: "Download Link to Disk" or "Save Target As" or "Save Link As").
